Vaccine Clinic

We will be offering flu and COVID-19 vaccinations to individuals with qualifying insurance plans. We will bill insurance for these vaccinations. Please see the attached list of accepted insurance plans.

We can offer flu vaccines at no cost to uninsured individuals. COVID-19 vaccines are not available at no cost to uninsured individuals, except for minors (18 years or younger).

Please bring a printed copy of your insurance card(s) and vaccination records, if you have them. Clients should call their insurance provider to verify coverage and any potential costs before receiving vaccines.

Clients with non-contracted insurance and/or pharmacy prescription cards should see their providers for their shots. If a client provides an insurance plan that we are unable to bill or that does not cover the vaccines, the client will be billed for the cost of the vaccines.
